Opening your MidBest Bank Health Savings Account is easy.
Here is what you’ll need to make the process move quickly.
Applicant
- Social Security Number
- Driver's License or State ID. If you have another type of ID, you will need to apply in person.
- Your contact information including an email address
- Your employer information
Signer(s)
Adding a signer allows you, the owner of the account to designate a person(s) to have signing authority only. A signer does not have ownership of the funds. Signers may only sign for the account until the owner's death or revocation of signatory rights. We will ask for the same information for the
signer as we do for the applicant. Signers are also eligible for a debit card.
Beneficiaries
- You must enter at least one beneficiary
- Legal name
- Date of birth
- Social Security Number
Transfer Form if You Currently Have an HSA
If you have been contributing to an HSA at another financial institution, we can
automatically transfer it to your new HSA hassle free. Once we have officially opened your new HSA, we will conduct the transfer on your behalf.
